The Application for Survey was designed to provide the Accreditation Review Committee (ARC) with a profile of the organization that has requested an accreditation survey. The answers to the questions do not weigh toward achieving or not achieving accreditation. They provide descriptive information, which is helpful to the surveyors, staff and Accreditation Review Committee in understanding the organization and its practices. Surveyors will review the application and supporting documents prior to conducting the on-site survey and may seek verification and clarification of certain items during or prior to the survey.
